This interdisciplinary partnership was funded by the SD Department of Health to implement several components of the South Dakota (SD) Diabetes Prevention and Control Plan (DPCP).  The team will identify disparities in diabetes risk, determine high risk groups, recognize the barriers to diabetes care, develop tools to lessen the burden of diabetes on those diagnosed, and construct educational guidelines for patients and health care professionals alike.
